  Background: In China the mean annual suicide rate is 23 per 100,000 and there are about 287,000 suicide deaths per year.Suicide, an important public health problem, seriously affects social and economic development in China.Suicide is not generated from a risk factor alone but resulted by social and psychological factors as well as the interactions between them.Many studies proved that self-esteem can predict suicide ideation, and low self-esteem was closely related to feelings of depression and hopelessness.Self-esteem can play an important role in suicide.However, few researches have been reported about the relationship between self-esteem and suicide deaths.Methods: Paired case-control study was used.25 towns in 3 counties of Shandong province were randomly selected.Case group was suicide cases happened from Sep.1, 2008 to Aug.31, 2009 inthose 25 towns.Controls were selected from normal persons matched with same age (no more than 3 years), same gender, and same or neighbour village as suicide cases.Data of 200 suicide deaths and 200 paired controls were collected.A structured questionnaire including demographics, familial environments, and personal characteristics was administered to the informants.Results: The Cronbachs α of suicides was 0.880, and the Cronbachs α of controls was 0.875, fluctuatien of Cronbachs α coefficient of SES after each item deleted was small.The correlation coefficients between the total scores of the SES and that of Trait Anxiety Inventory (TAI) in suicides and controls were-0.509 and-0.564, respectively (P<0.01).The factor analysis showed that two factors extracted from the SES scale in two groups which could explain 61.700% and 63.802% of total variance, respectively.There was significant difference between suicide and control groups in SES score (t=17.525, P<0.01).The odds ratio (OR) of self-esteem associated with suicide was 0.880 (95%Cl=0.818~0.947).Conclusion: SES has high reliability and validity, and its worth being applied in rural suicide study in China.Self-esteem is negatively associated with suicide in rural population.
