患者女,30岁。发现左上腹无痛性肿块半月余,无腹痛、腹泻及畏寒发热,腰部有轻度酸胀不适,大小便正常。体检:左上腹可扪及约10cm×15cm大小包块,质中,界限欠清楚,表面尚光滑,不活动,有轻度触痛。实验室检查及胸部X线摄片均无明显异常,CT检查:见左侧腹膜后左肾前上方有一约10cm×12cm巨大肿块,中心有不规则的低密度区,CT值15～55Hu,边界较光整。左肾受压向后推移,胰腺明显推向左前方。增强后肿块实质部分有强化,中心仍显示为低密度区。左肾上腺显示不清。主动脉旁及后腹膜淋巴结未见肿大。手术所见:肿块位于胰腺后方,将胰腺及胃向前顶起,约15cm×10cm×8cm大小,质地中等,打开后腹膜见肿块表面均为扩张的血管,腹腔动脉亦受压增粗, Female patient, 30 years old. CT examination showed a large mass of about 10cm x 12cm in the upper left of the left peritoneum and left kidney. The center had an irregular low-density area with a CT value of 15 to 55 Hu. More light than the whole. The left kidney was pushed back and the pancreas was pushed forward to the left. After the enhancement, the substantial part of the mass was strengthened and the center still showed a low density area. The left adrenal gland appears ill. Para-aortic and retroperitoneal lymph nodes were not enlarged. Surgical findings: The mass is located behind the pancreas and pushes the pancreas and stomach forward. The size is about 15cm x 10cm x 8cm. The texture is medium. After the peritoneum is opened, the peritoneal surface shows dilated blood vessels. The celiac artery is also thickened by pressure.
