目的评估老年人群中实行甲状腺全切除术治疗多结节性甲状腺肿的术中安全性及长期结局。方法根据手术时年龄对10年中232例实施甲状腺全切除术的多结节甲状腺肿患者进行回顾性分析。结果老年患者组中(>70岁),手术时间(P=0.023)、术中出血(P=0.03)、切除的甲状腺重量(P<0.001)和胸骨后肿块的比例(P<0.001)显著增高;而外科相关并发症的发生率,包括喉返神经麻痹和甲状旁腺功能减退发生率相似。老年组中术后肺炎发生率较多(P=0.34)。老年组中并发症的数目与住院时间和生存时间有关。结论老年多结节性甲状腺肿患者实施甲状腺全切除术时,术中结果与年轻患者相似,但是他们的长期结局可能受到术前并发症种类的影响。
Objective To evaluate the safety and long-term outcome of total thyroidectomy in the elderly population for treatment of multiple-nodular goiter. Methods According to the age of the operation, 232 cases of multi-nodular goiter who underwent total thyroidectomy in 10 years were retrospectively analyzed. Results The age (> 70 years), operation time (P = 0.023), intraoperative bleeding (P = 0.03), thyroid weight resected (P <0.001) and the ratio of the sternum (P <0.001) The incidence of surgical-related complications, including recurrent laryngeal paralysis and hypoparathyroidism, were similar. The incidence of postoperative pneumonia in the elderly group was higher (P = 0.34). The number of complications in the elderly group was related to length of stay and hospital stay. Conclusions The results of total thyroidectomy in elderly patients with multiple nodular goiter are similar to those in younger patients, but their long-term outcome may be influenced by the type of preoperative complications.
