目的观察牙半切除术联合纤维桩核修复治疗下颌磨牙病变的临床效果。方法 53例共53颗牙根受累的下颌磨牙患者,经完善根管治疗后行牙半切术,术后3个月采用D.T.Light Post纤维桩、DUO-LINKTM双固化树脂黏接剂、Light-Core光固化树脂核材料恢复桩核,应用钴铬合金烤瓷冠修复,评估术后修复体临床疗效。结果本组9例行邻牙联冠修复,其保留牙根的牙槽骨吸收均达根长1/2,无松动,5例下颌第一磨牙中2例保留近中根,行相邻下颌67联冠修复,3例保留远中根,行相邻下颌56联冠修复,4例下颌第二磨牙行相邻67联冠修复;44例行单冠修复,其保留牙根的牙槽骨吸收至根颈1/3,邻牙及对牙合牙均无异常。其中31例下颌第一磨牙21例保留近中根,10例保留远中根,13例下颌第二磨牙8例保留近中根、5例保留远中根;随访1~3a,成功46例,有效3例,失败4例。结论 牙半切除术联合纤维桩核修复治疗下颌磨牙病变效果满意。
Objective To observe the clinical effect of tooth hemi-resection and fiber post-core repair for the treatment of mandibular molars. Methods A total of 53 patients with mandibular molar suffering from root failure were enrolled in this study. After complete root canal treatment, they were subjected to half-mastectomy. The adjacent mandibular 56-crown was repaired in 4 cases, and the second mandibular second molar in 67 cases. 1/3, adjacent teeth and teeth are no abnormalities. Among them, 31 cases of mandibular first molars retained proximal root, 10 cases retained distal roots, 13 cases of mandibular second molars retained proximal roots and 5 cases retained distal roots. The follow-up ranged from 1 to 3 years, 46 cases were successful and 3 cases were effective. Failure in 4 cases. Conclusion The effect of partial hemiarthroplasty and fiber post-and-core repair on mandibular molars is satisfactory.
