目的:探讨Adams-Berger法韧带重建治疗慢性桡尺远侧关节不稳的临床应用和疗效。方法:自2014年1月至2018年6月，我们采用Adams-Berger法韧带重建治疗慢性桡尺远侧关节不稳定患者11例。术中于桡骨远端尺背侧钻孔，将切取的掌长肌腱穿过桡骨隧道；并在尺骨颈和茎突隐窝之间钻孔，将肌腱两个断端经尺骨隧道引至尺骨颈处，前臂置于中立位，克氏针固定，抽紧缝合掌长肌腱。结果:术后所有切口Ⅰ期愈合。11例患者均获得随访，时间为6～12个月，平均8个月。所有患者疼痛均缓解，并恢复工作。采用改良Mayo评分评估腕关节功能：优8例，良3例。VAS评分由术前的(4.5±1.2)分改善至术后的(0.4±0.6)分。结论:Adams-Berger法韧带重建治疗慢性桡尺远侧关节不稳定，术后腕关节功能改善明显。“,”Objective:To explore the clinical application and efficacy of Adams-Berger ligament reconstruction for the treatment of chronic distal radioulnar joint (DRUJ) instability.Methods:From January 2014 to June 2018, 11 patients with chronic DRUJ instability were treated with Adams-Berger ligament reconstruction. During the operation, a bone tunnel was drilled at the dorsal ulnar side of the distal radius. The long palmar tendon was cut and put through the radius tunnel. And a hole was drilled between ulnar neck and styloid process recess. The two broken ends of the tendon were led to the ulnar neck through the ulna tunnel. The forearm was placed in the neutral position, fixed by Kirschner wire, and the long palmar tendon was sutured tightly.Results:All the incisions achieved primary healing. All the 11 patients were follow-up for 6 to 12 months with an average of 8 months. All the patients were relieved of pain and returned to work. The wrist function was evaluated by modified Mayo score and the results were excellent in 8 cases and good in 3 cases. The VAS score was improved from (4.5±1.2) before operation to (0.4±0.6) after operation.Conclusion:The application of Adams-Berger ligament reconstruction for the treatment of chronic DRUJ instability can significantly improve the function of wrist joint.
