目的:研究血清心肌酶谱及心肌肌钙蛋白I(c Tn I)对新生儿缺氧缺血性脑病的诊断价值。方法:选取2012年11月到2014年11月我院收治的新生儿缺氧缺血性脑病患儿70例(研究组),另选同期健康新生儿70例(对照组),采用生化仪检测入选者血清心肌酶谱,应用化学发光法检测血清c Tn I,比较两组血清心肌酶谱和c Tn I水平。结果:研究组肌酸激酶(CK)、天门冬氨酸氨基转移酶(AST)、肌酸激酶同工酶(CK-MB)、a-羟丁酸脱氢酶(HBDH)、乳酸脱氢酶(LDH)均显著高于对照组,两组比较差异具有统计学意义(P<0.05);研究组c Tn I水平显著高于对照组,两组比较差异具有统计学意义(P<0.05);CK-MB检测灵敏度最高,c Tn I检测的特异性最高。结论:血清心肌酶谱及c Tn I对新生儿缺氧缺血性脑病具有重要的诊断价值,为临床治疗提供一定依据。
Objective: To investigate the value of serum myocardial enzymes and cardiac troponin I (cTn I) in the diagnosis of neonatal hypoxic-ischemic encephalopathy. Methods: From November 2012 to November 2014, 70 children with hypoxic-ischemic encephalopathy (study group) and 70 healthy newborn children (control group) were enrolled in our hospital. Biochemical analyzer Serum enzyme of myocardial enzymes were determined by chemiluminescence method for detection of serum cTnI, serum myocardial enzymes and cTnI levels were compared between the two groups. Results: In the study group, creatine kinase (CK), aspartate aminotransferase (AST), creatine kinase isoenzyme (CK-MB), a-hydroxybutyrate dehydrogenase (HBDH), lactate dehydrogenase (P <0.05). The level of cTnI in study group was significantly higher than that in control group (P <0.05). There was significant difference between the two groups (P <0.05). CK-MB detection of the highest sensitivity, cTnI detection of the highest specificity. Conclusion: Serum myocardial enzymes and cTnI have important diagnostic value in neonatal hypoxic-ischemic encephalopathy and provide some evidences for clinical treatment.
