目的 探讨硬质支气管镜 (硬镜 )在临床上的应用。方法 对 2 0 0 1- 0 1~ 2 0 0 4 - 0 8广东省人民医院呼吸科 182例患者硬镜的临床应用进行分析报道。其中男性 115例 ,女性 6 7例。年龄 19~ 88(5 7± 2 2 )岁。包括气道内放置支架 14 4例 ,气道内激光治疗 2 9例 ,大咯血处理 6例 ,气道内巨大异物取出 3例。全部患者均根据要求在上述介入操作前应用全身麻醉后置入德国STORZ公司成人通用硬镜。结果 182例患者中 177例成功一次置入硬镜 ,3例多次均未成功置入硬镜 ,最后通过经纤维支气管镜引导成功置入 ,2例患者因不能成功置入硬镜而放弃。 180例置入硬镜的患者 ,术后副作用包括 :咽喉痛及烧灼感 (98 90 % )、喉头水肿 (1 6 5 % )、纵隔气肿及右侧气胸 (0 5 5 % )、牙损伤脱落 (1 0 9% )、操作时患者不配合致留置针脱落 (0 5 5 % )。这些并发症影响轻微 ,经适当处理后均短期内消失。结论 硬质支气管镜是介入性肺病学技术的重要操作工具 ,可以达到安全、有效、并发症少的目的。
Objective To investigate the clinical application of rigid bronchoscope (hard mirror). Methods The clinical data of 182 cases of patients with respiratory diseases in Guangdong Province People’s Hospital from 2000-01 to 2004-04 were analyzed. There were 115 males and 67 females. Age 19 ~ 88 (57 ± 2 2) years old. Including 14 cases of stent placement in the airway, 29 cases of laser treatment in the airway, 6 cases of massive hemoptysis and 3 cases of huge foreign body in the airway. All patients were required to be placed in Germany STORZ company adult general purpose hard mirror before the above intervention before general anesthesia. Results One hundred and seventy-seven of 182 patients were successfully treated with one-time hardcopy. Three of them were unsuccessfully placed in the scleral mirror many times. Finally, they were successfully guided by fiberoptic bronchoscopy and two patients were abandoned because they could not be successfully inserted into the scleral mirror. Among the 180 patients who underwent hardcopy, postoperative side effects included sore throat and burning sensation (98 90%), laryngeal edema (165%), mediastinal emphysema and right pneumothorax (0.55%), tooth injuries Shedding (109%). The patient did not fit the indwelling needle during operation (0 5 5%). These complications have minor effects and disappear shortly after proper treatment. Conclusions The rigid bronchoscope is an important operating tool for interventional pulmonary disease, which can achieve the goal of safety, efficacy and fewer complications.
