研究高血压病人的钠负荷时肾脏多巴胺(DA)生成量的变化。方法7例年龄56~65岁的高血压病患者(A组)、7例年龄匹配的正常人(B组)以及10例年青健康自愿者(C组)在代谢平衡状态下将摄钠量从34mmol/d增至170mmol/d。观察各组血尿DA、Ccr等变化。结果B组和C组增加钠负荷后尿DA均明显增加,但B组尿DA的增幅小于C组。与此相反,高血压病患者钠负荷后尿DA最初降低,而后逐渐升至基础值。各组尿钠的变化与尿DA平衡。三组血浆DA、Ccr和平均动脉压均无变化,但A组钠负荷后血钠增高,血细胞压积降低,体重增加量大于对照组。结论肾脏DA生成能力与年龄有关。高血压病患者在增加钠负荷时,肾脏不能相应增加DA的生成,这一缺陷可能与高血压病时肾脏排钠异常有重要关系
To study the changes of renal dopamine (DA) production in hypertensive patients during sodium load. Methods Seven patients aged 56-65 years old with hypertension (group A), 7 age-matched normal subjects (group B) and 10 young healthy volunteers (group C) 34 mmol / d to 170 mmol / d. Observe hematuria DA, Ccr and other changes in each group. Results The urinary DA in group B and C increased significantly after sodium loading, but the increase in urine DA in group B was less than that in group C. In contrast, sodium overload in patients with hypertension initially decreased DA and then gradually rose to baseline values. Urinary sodium changes in each group with urinary DA balance. There was no change in plasma DA, Ccr and mean arterial pressure in the three groups, but the sodium level in group A was increased, the hematocrit decreased, and the weight gain was greater than that in the control group. Conclusions Kidney DA production is related to age. Hypertension patients with increased sodium load, the kidneys can not be a corresponding increase in the generation of DA, this defect may be associated with renal abnormality in hypertension when sodium is important
