本文报告了9例伴有关节症状的多发性肌炎和皮肌炎,并叙述了其关节表现的临床和病理的资料。这9例病人,女性7例,男性2例;年龄20～63岁,均不伴发肿瘤,其中3例有皮疹。关节症状出现的时间:有5例在肌炎症状前1～3月出现,另4例与肌炎同时发生。侵犯的部位以掌指、近端指间、腕和膝关节为主。经X线摄片检查,8例均仅有软组织肿胀,故为轻度非侵蚀性关节炎,对类固醇激素(强的松20～80mg/天不等)治疗反应良好,经治后关节症状均在一周内消失。另1例为侵蚀性关节炎(指间关节显示关节边缘侵蚀)和关节周围钙化,经应用秋水仙碱后症状减轻。本组病人均有肺部受累的表现,包括间质纤维化、一氧化碳弥散能力降低、低氧症等。所有病人类风湿因子和抗核抗体试验均阴性。
This article reports on 9 cases of polymyositis and dermatomyositis with joint symptoms and describes the clinical and pathological data on their joint manifestations. The 9 patients, 7 females, 2 males; aged 20 to 63 years, were not associated with tumors, including 3 cases of rash. The other 1 case was erosive arthritis (interphalangeal joint showed edge erosion) and periarterial calcification, and the symptoms were relieved after applying colchicine. This group of patients have lung involvement, including interstitial fibrosis, carbon monoxide reduced ability to hypoxia and so on. All patients with rheumatoid factor and antinuclear antibody test were negative.
