目的:分析影响显微手术切除前颅窝脑膜瘤手术效果的因素。方法:回顾性分析南京医科大学附属淮安第一医院于2011年7月~2013年7月采用显微手术切除的43例前颅窝脑膜瘤患者的临床资料,分析影响肿瘤残留、术后视力改善及并发症的相关因素,然后对有统计学意义的因素行Logistic逐步回归分析,研究其各自的独立危险因素。结果:肿瘤的形状及侵袭性是影响肿瘤残留的独立危险因素;肿瘤大小及神经血管包绕是影响术后视力改善的独立危险因素;神经血管包绕及侵袭性是影响术后并发症的独立危险因素。结论:前颅窝脑膜瘤患者的肿瘤大小、形状、神经血管包绕、侵袭性是肿瘤残留、术后视力改善及并发症的重要因素,能够为手术效果提供良好的预测价值。
Objective: To analyze the factors influencing the operative effect of microsurgical anterior fossa meningiomas. Methods: The clinical data of 43 cases of anterior fossa meningiomas resected by microsurgical operation from July 2011 to July 2013 in Huai’an First Affiliated Hospital of Nanjing Medical University were retrospectively analyzed. The changes of residual tumor, visual acuity after operation And complications, and then statistically significant factors Logistic stepwise regression analysis of their respective independent risk factors. Results: The shape and invasiveness of tumor were independent risk factors affecting tumor residual. Tumor size and neurovascular wrapping were the independent risk factors for postoperative visual acuity. The involvement of neurovascular wrapping and invasiveness was independent of postoperative complications Risk factors. CONCLUSION: Tumor size, shape, neurovascular wrap and invasiveness in patients with anterior fossa meningioma are the important factors of tumor residual, postoperative visual acuity improvement and complications, which can provide good predictive value for the surgical results.
