目的探讨空腹血糖(FPG)、糖化血红蛋白(HbA1c)在重症颅脑外伤急性期病情评估及预后判定中的价值。方法收集56例重症颅脑损伤患者展开研究,根据格拉斯哥昏迷评分(GCS)、FPG与HbA1c结果进行分组,对比重度颅脑损伤组和极重度颅脑损伤组、应激性高血糖组和糖尿病高血压组、不同预后患者的LDH、FPG水平变化情况。结果极重度颅脑损伤组与重度颅脑损伤组患者的FPG、LDH水平存在明显差异,前者明显大于后者,恢复较好组和预后差及死亡组患者的LDH、FPG水平存在明显差异(P<0.05)。结论 FPG指标水平可以作为评定患者颅脑损伤程度及预后效果的重要指标,HbA1c指标对诊断糖尿病或应激性血糖增高病症具有重要意义。
Objective To investigate the value of fasting blood glucose (FPG) and glycosylated hemoglobin (HbA1c) in the assessment and prognosis of acute traumatic brain injury. Methods 56 patients with severe craniocerebral injury were enrolled in this study. According to Glasgow Coma Scale (GCS), FPG and HbA1c results, the patients with severe craniocerebral injury group and severe craniocerebral injury group, stress hyperglycemia group and high diabetes mellitus group Blood pressure group, different prognosis of patients with LDH, FPG level changes. Results The levels of FPG and LDH in patients with severe traumatic brain injury and those with severe traumatic brain injury were significantly different. The former was significantly higher than the latter. The levels of LDH and FPG in patients with poor prognosis and poor prognosis were significantly different (P <0.05). Conclusion The level of FPG can be used as an important index in assessing the degree of brain injury and prognosis of patients. HbA1c is important for the diagnosis of diabetes or stress-induced hyperglycemia.
