目的依据美国临床实验室标准化协会(CLSI)EP7-A2文件评价溶血和乳糜血对凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)测定的干扰。方法按照“配对差异”方案,配置含不同浓度血红蛋白(Hb)和不同浊度的乳糜的正常及高值PT和APTT标本,进行干扰评价试验。结果正常水平的PT和APTT不受Hb和乳糜浊度的干扰。当Hb≥2.55g/L时,Hb对高值APTT测定产生负干扰,并呈线性函数关系,Hb≥10.20g/L时对高值PT有干扰;当乳糜浊度≥745FTU时,乳糜血对高值PT测定产生正干扰,呈线性函数关系;高值APTT测定不受乳糜血干扰。结论中度溶血对高值APTT干扰明显,重度溶血对高值PT测定有干扰;中度乳糜血对高值PT干扰明显;正常水平的PT和APTT不受溶血和乳糜血的干扰,高值APTT不受乳糜血干扰。
Objective To evaluate the effect of hemolysis and chylous blood on prothrombin time (PT) and activated partial thromboplastin time (APTT) determination according to CLSI EP7-A2. Methods The normal and high PT and APTT specimens with different concentrations of hemoglobin (Hb) and chylous turbidity were prepared according to the “Paired Differences” protocol. Results Normal levels of PT and APTT were not disturbed by Hb and chyle turbidity. When Hb≥2.55g / L, Hb negatively interfered with the high APTT assay, and showed a linear function. When Hb≥10.20g / L, it interfered with high PT. When the chyle turbidity≥745FTU, High-value PT measurement produced positive interference, as a linear function; high value APTT assay was not affected by chymalic interference. Conclusions Moderate hemolysis has obvious interference with high value APTT. Severe hemolysis has interference with high value PT. Intermittent moderate and high concentration PT can interfere with PT. Normal level of PT and APTT are not affected by hemolysis and chylomicrons, and high value APTT Not affected by chymalic interference.
