目的 研究空腹甘油三酯水平 (TG)正常的 2型糖尿病患者餐后脂代谢的改变。方法采用改良脂肪餐负荷试验对 33例空腹TG正常 (观察组 )和 10例空腹TG增高 (高脂组 )的 2型糖尿病患者以及 10名健康对照者 (对照组 )进行餐后脂代谢研究。前者依据餐后 4小时TG(TG4h)再分为餐后正常组和餐后增高组。结果 脂餐后受试者 2~ 8小时TG水平均有所增高 ,高密度脂蛋白 胆固醇 (HDL C)都有一个轻微的低谷改变 ,其中观察组较对照组TG4h的增高和 4小时HDL C的降低差异有显著性 (P <0 .0 5 ) ;餐后总胆固醇 (TC)变化均不明显 ,组间各时段比较差异无显著性 (P >0 .0 5 )。餐后TG峰值 ,TG曲线下面积 (TG AUC)和TG峰值时间按对照组、餐后正常组、餐后增高组和空腹增高组的次序依次递增 (前两组比较和后两组比较均为P >0 .0 5 )。结论 部分空腹TG水平正常的糖尿病患者存在餐后TG水平的增高、廓清延迟和HDL C降低 ,其程度与空腹高TG血症者基本相当
Objective To study the change of postprandial lipid metabolism in type 2 diabetic patients with normal fasting triglyceride level (TG). Methods A modified fat meal test was used to study the postprandial lipid metabolism of 33 patients with fasting TG normal (observation group) and 10 fasting TG patients with hyperlipidemia (type 2 diabetes mellitus) and 10 healthy controls (control group). The former is divided into normal postprandial group and postprandial hyperplasia group according to 4 hours postprandial TG (TG4h). Results The level of triglyceride (TG) increased from 2 to 8 hours after fat meal, and there was a slight change in HDL cholesterol. The increase of TG4h and the increase of HDL C in 4 hours in observation group (P <0.05). There was no significant difference in postprandial total cholesterol (TC) between the two groups (P> 0.05). The postprandial TG peak, the area under the TG curve (TG AUC) and the peak time of TG were increased in the order of the control group, normal postprandial group, postprandial hyperxia group and fasting hyperplasia group (the first two groups were compared and the latter two groups were P> 0 .0 5). Conclusion Some TG patients with normal fasting TG levels had elevated postprandial TG levels, delayed debridement and decreased HDL C levels, which were almost the same as fasting TG patients
