目的综述β-内酰胺类抗生素交叉过敏反应的结构基础。方法结合笔者近年来研究,综合国内外有关β-内酰胺类抗生素交叉过敏反应的文献,评价该类药物间交叉过敏反应的结构基础。结果与结论青霉素类之间、头孢菌素类之间和两类抗生素之间均存在部分交叉过敏反应,识别部位如侧链在交叉过敏反应中起重要作用。当药物共有结构如母核成为识别位点时,交叉过敏反应发生率较高;当药物侧链成为识别位点时,具有相同或相似侧链的药物间可能发生交叉过敏反应;而当药物特有结构成为抗体识别位点时,可能仅对该药发生过敏反应,即特异性过敏反应。
Objective To summarize the structural basis of the cross-allergic reaction of β-lactam antibiotics. Methods Based on the author’s research in recent years, combining the domestic and foreign literature on the cross-allergic reaction of β-lactam antibiotics to evaluate the structural basis of cross-allergic reactions among these drugs. RESULTS AND CONCLUSIONS Partial cross-allergic reactions exist between penicillins, cephalosporins and two types of antibiotics. Recognition sites such as side chains play an important role in the cross-allergic response. When the common structure of the drug, such as mother nucleus, becomes a recognition site, the incidence of cross-allergic reaction is high; when the drug side chain becomes a recognition site, cross-allergic reactions may occur between drugs with the same or similar side chains; and when drug- When the structure becomes an antibody recognition site, it may be allergic to the drug only, ie, a specific allergic reaction.
