目的:观察枯草杆菌二联活菌颗粒(妈咪爱)联合非营养性吸吮(NNS)治疗早产儿喂养不耐受的临床疗效。方法:选取喂养不耐受早产儿54例,随机分为治疗组26例和对照组28例,两组均给予暖箱保暖、部分静脉营养支持、预防感染、早期微量喂养或鼻饲,治疗组加服枯草杆菌二联活菌颗粒(每次0.5 g,2次/日,连用7 d)联合非营养性吸吮(喂奶前后给予早产儿型硅胶安抚奶嘴吸吮10~15 min)治疗。观察两组呕吐、腹胀消失时间、达全胃肠喂养时间及恢复至出生体重时间、总体疗效及并发症发生情况。结果:治疗组总有效率96.15%,高于对照组的78.57%(P<0.01);与对照组比较,治疗组呕吐、腹胀消失时间缩短,胃潴留量减少,达全胃肠喂养时间及恢复至出生体重时间缩短,并发症发生率下降,差异均有统计学意义(P<0.01)。结论:枯草杆菌二联活菌颗粒联合非营养性吸吮治疗早产儿喂养不耐受疗效显著,值得临床推广应用。
Objective: To observe the clinical efficacy of Bacillus subtilis and Bacillus subtilis (Mommy Love) combined with non-nutritive sucking (NNS) in the treatment of preterm infants intolerance. Methods: 54 cases of preterm infants who were intolerant to feeding were randomly divided into treatment group (n = 26) and control group (n = 28). Both groups were given warm box, partial parenteral nutrition support, prevention of infection, early micronutrient or nasal feeding, B. subtilis B. subtilis granules (0.5 g twice daily for 7 days) combined with non-nutritive sucking (prenatal and postnatal suck for 10-15 min). The two groups were observed vomiting, abdominal distension disappeared, up to the total gastrointestinal feeding time and return to birth weight time, the overall efficacy and complications. Results: The total effective rate of the treatment group was 96.15%, which was higher than that of the control group (78.57%, P <0.01). Compared with the control group, the treatment group had shorter vomiting, shorter disappearance of abdominal distension, less gastric retention, total gastrointestinal feeding time and recovery Time to birth weight shortened, the incidence of complications decreased, the differences were statistically significant (P <0.01). Conclusion: The combination of Bacillus subtilis viable granules and non-nutritive sucking treatment of preterm infants with intolerance has a significant therapeutic effect and is worthy of clinical application.
