目的 :评估外周血白细胞层结核DNA定性检测在肺结核诊断中的应用价值 ,寻找肺结核新的诊断手段。方法 :根据肺结核不同的临床表现 ,将 1 4 4例肺结核患者分为Ⅱ型肺结核痰结核分枝杆菌阳性组和阴性组及Ⅲ型肺结核痰结核分枝杆菌阳性组和阴性组。分别取静脉血 5ml进行白细胞层结核DNA定性检测 ,利用健康人静脉血作对照以对本方法进行质量控制。结果 :4 7例血行播散型肺结核组外周血白细胞层结核DNA总阳性率为 6 5 .9% ;其中 1 9例痰结核分枝杆菌阳性的患者结核DNA阳性 1 3例 ,阳性率为 6 8% ,2 8痰结核分枝杆菌阴性患者结核DNA阳性 1 8例 ,阳性率为 6 4%。 97例继发性肺结核组外周血白细胞层结核DNA阳性 6 4例 ,阳性率为 6 6 % ;其中 4 3例继发性肺结核痰结核分枝杆菌阳性组结核DNA阳性 2 8例 ,阳性率为 6 5 % ,5 4例继发性肺结核痰结核分枝杆菌阴性患者的结核DNA阳性 36例 ,阳性率为 6 7%。总之 ,肺结核组外周血白细胞层结核DNA总的阳性率为6 6 % ;痰结核分枝杆菌阳性组的阳性率为 6 6 % ;痰结核分枝杆菌阴性组的阳性率为 6 5 .8%。上述各组之间比较无统计学差异 ,P >0 .0 5。健康人组均为阴性。结论 :肺结核患者外周血白细胞层结核DNA定性检测阳性率为 6 6 % ,不受痰结核分枝杆菌检出率的影响。外周血?
Objective: To evaluate the value of qualitative detection of peripheral blood leukocyte layer DNA in the diagnosis of pulmonary tuberculosis and to search for new diagnostic tools for pulmonary tuberculosis. Methods: According to the different clinical manifestations of tuberculosis, 144 cases of pulmonary tuberculosis patients were divided into two groups: positive and negative group of sputum Mycobacterium tuberculosis type Ⅱ and group of positive and negative group of sputum Mycobacterium tuberculosis of type Ⅲ pulmonary tuberculosis. Were taken venous blood 5ml qualitative detection of leukocyte layer of DNA, the use of healthy human venous blood as a control to the quality control of the method. Results: The total positive rate of TB DNA in peripheral blood leukocyte from 47 cases of hematogenous disseminated pulmonary tuberculosis was 65.9%. Among them, 13 cases of tuberculosis DNA were positive in 19 cases of sputum positive Mycobacterium tuberculosis, the positive rate was 6 8%, 28 tuberculosis sputum negative specimens of tuberculosis DNA positive in 18 cases, the positive rate was 64%. 97 cases of secondary tuberculosis group of peripheral blood leukocyte layer of tuberculosis DNA positive of 64 cases, the positive rate was 6 6%; of which 43 cases of secondary tuberculosis sputum Mycobacterium tuberculosis positive tuberculosis DNA positive 28 cases, the positive rate was Sixty-five (54%) cases of tuberculosis DNA were negative in sputum of Mycobacterium tuberculosis in 54 cases, the positive rate was 67%. In conclusion, the positive rate of tuberculin DNA in peripheral blood leukocyte layer of tuberculosis group was 66%, that of sputum Mycobacterium tuberculosis positive group was 66%, that of sputum Mycobacterium tuberculosis negative group was 65.8% . There was no significant difference between the above groups, P> 0.05. Healthy people were negative. Conclusion: The positive rate of qualitative DNA detection of peripheral blood leukocyte in tuberculosis patients is 66%, which is not affected by the detection rate of Mycobacterium tuberculosis.
