目的探讨血清抗人型纯化蛋白衍生物(purified protein derivative,PPD)Ig G抗体对活动性肺结核的诊断价值。方法 2013年6月—2014年3月以ELISA法测定活动性肺结核组(病例组)32例及健康对照组(对照组)40例血清抗PPD Ig G特异性抗体水平,计量资料采用t检验,P<0.05为差异有统计学意义。以受试者工作特征曲线(receiver operating characteristic curve,ROC)分析法评价其作为诊断试验的价值。结果病例组血清抗PPD Ig G抗体水平明显高于对照组[(2.832±1.578)、(1.297±1.109)],差异有统计学意义(P<0.05)。ROC曲线下面积为0.797,95%置信区间为(0.643,0.950),以x±1.0 s作为诊断界点时敏感度最高,特异度也较高。结论血清中抗PPD Ig G抗体对活动性肺结核有一定的诊断价值。
Objective To investigate the diagnostic value of serum anti-human purified protein derivative (PPD) Ig G antibody in patients with active pulmonary tuberculosis. Methods Thirty-two patients with active pulmonary tuberculosis (case group) and 40 healthy controls (control group) were assayed for serum anti-PPD Ig G-specific antibodies by ELISA method from June 2013 to March 2014. The measurement data were analyzed by t test, P <0.05 for the difference was statistically significant. Receiver operating characteristic curve (receiver operating characteristic curve, ROC) analysis of evaluation as a diagnostic test value. Results The level of serum anti-PPD Ig G antibody in the case group was significantly higher than that in the control group [(2.832 ± 1.578), (1.297 ± 1.109)], the difference was statistically significant (P <0.05). The area under the ROC curve was 0.797, the 95% confidence interval was (0.643, 0.950). The highest sensitivity and specificity were found when using x ± 1.0 s as the diagnostic endpoint. Conclusion Serum anti-PPD Ig G antibody has certain diagnostic value for active pulmonary tuberculosis.
