目的观察重组乙型肝炎(乙肝)疫苗(汉逊酵母)[Hepatitis B Vaccine Made by Recombinant Deoxyribonucleic Acid Techniquesin Hansenula Polymorpha Yeast,HepB-HPY]在藏族易感人群中的免疫应答,探讨其影响因素。方法为1609名从横断面调查中筛选出的乙肝病毒(Hepatitis B Virus,HBV)易感者,接种HepB-HPY,剂量10微克/毫升(μg/ml),免疫程序0、1、6个月。首剂免疫7个月后,随机整群选择615名受种者填写调查表,采集静脉血标本,用固相放射免疫测定法定性检测乙肝病毒表面抗原(HBV Surface Antigen,HBsAg)、抗乙肝病毒核心抗原抗体,用Abbott公司的微粒子酶免疫试剂定量检测抗乙肝病毒表面抗原抗体(Antibody to HBsAg,Anti-HBs)。结果共有549名受种者Anti-HBs阳转,阳转率89.3%,阳转者Anti-HBs几何平均浓度(Geometric Mean Concentration,GMC)为3514.6毫国际单位/毫升(mIU/ml)。通过Logistic回归分析,Anti-HBs阳转率和GMC均呈随年龄增大而降低的趋势,而与受种者的性别、职业、居住地等无关。结论藏族易感人群对HepB-HPY有良好的免疫应答。Anti-HBs阳转率和GMC均与受种者的年龄有关,而与受种者的性别、职业、居住地等无关。
Objective To observe the immune response of Hepatitis B Vaccine Made by Recombinant Deoxyribonucleic Acid Techniques in Hansenula Polymorpha Yeast (HepB-HPY) in Tibetan susceptible population and to explore the influencing factors. METHODS: One hundred and sixty-nine susceptible individuals to Hepatitis B Virus (HBV) selected from a cross-sectional survey were vaccinated with HepB-HPY at a dose of 10 μg / mL for 0, 1, and 6 months . Seven months after the first dose of immunization, 615 random subjects were randomly selected to fill in the questionnaire and venous blood samples were collected. HBV surface antigen (HBsAg), hepatitis B virus Antibody to HBsAg (Anti-HBs) was quantitated with Abbott microparticle enzyme immunoassay. RESULTS: A total of 549 recipients were positive for Anti-HBs with a positive conversion rate of 89.3% and a Geometric Mean Concentration (GMC) of 3514.6 mIU / ml. Logistic regression analysis showed that the anti-HBs positive rate and GMC decreased with age, but not with the sex, occupation and residence of the recipient. Conclusion Tibetan susceptible population has a good immune response to HepB-HPY. Anti-HBs positive rate and GMC are related to the age of the recipient, regardless of the recipient’s sex, occupation, place of residence and so on.
