目的探讨手术中应用缓释氟尿嘧啶植入剂(中人氟安)对进展期直肠癌的治疗效果。方法 46例(2006年1月至2008年3月)进展期直肠癌患者行直肠癌根治性切除,同时术中予氟尿嘧啶注射液600mg病变肠腔内注入,局部植入氟尿嘧啶缓释剂500mg,作为治疗组;43例(2003年1月至2007年1月)进展期直肠癌患者行直肠癌根治性切除术,术中予氟尿嘧啶注射液600mg病变肠腔内注入,作为对照组。2组患者一般资料相似,术后化疗方案相同。比较2组2年内生存率、局部复发率及远处转移率。结果治疗组与对照组2年内局部复发率分别为4.3%、20.9%(P<0.05);远处转移率分别为17.4%、20.9%(P>0.05);2年生存率分别为93.5%、79.1%(P<0.05)。结论术中应用缓释氟尿嘧啶植入剂是提高进展期直肠癌生存率及降低局部复发率的有效方法。
Objective To investigate the therapeutic effect of extended-release fluorouracil (Fluorouracil) on advanced rectal cancer during surgery. Methods Forty-six patients with advanced rectal cancer underwent radical resection of rectal cancer from January 2006 to March 2008, meanwhile 600 mg of fluorouracil injection was administered intra-artially into the intestine and 500 mg of fluorouracil sustained- Treatment group; 43 cases (January 2003 to January 2007) advanced rectal cancer patients underwent radical resection of colorectal cancer, intraoperative infusion of fluorouracil 600mg intralesional injection as the control group. Two groups of patients with similar general information, the same postoperative chemotherapy. Two groups of 2-year survival rate, local recurrence rate and distant metastasis rate were compared. Results The local recurrence rates in treatment group and control group within 2 years were 4.3% and 20.9% (P <0.05), respectively. The distant metastasis rates were 17.4% and 20.9% (P> 0.05) respectively. The 2-year survival rates were 93.5% 79.1% (P <0.05). Conclusion Intraoperative application of sustained release fluorouracil implant is an effective method to improve the survival rate of advanced rectal cancer and reduce the local recurrence rate.
