目的探讨脑电双频指数(B IS)指导丙泊酚用于小儿扁桃体摘除术对血流动力学影响及术后恢复情况。方法择期行扁桃体摘除术小儿60例,随机分成B IS监测丙泊酚组(B IS组)和对照组。B IS组(30例)术中通过维持B IS在(50±5)左右来调整丙泊酚用量大小,对照组根据术中患儿的血流动力学反应、呼吸和体动反应来调整丙泊酚用量大小,观察记录术中循环情况、B IS值、丙泊酚用量,清醒拔管时间及术后副反应情况。结果 B IS组麻醉过程及复苏期间血压及B IS值均较高,丙泊酚用量减少,清醒拔管时间缩短,术后恶心呕吐少。结论 B IS监测丙泊酚用于小儿扁桃体摘除术能保持围术期血流动力学稳定,减少丙泊酚用量,术后副反应少,加快麻醉恢复。
Objective To investigate the effect of propofol on the hemodynamics and postoperative recovery of pediatric tonsillectomy guided by bispectral index (BIS). Methods Totally 60 children with tonsillectomy were randomly divided into B IS monitoring propofol group (B IS group) and control group. In the B IS group (30 cases), the dosage of propofol was adjusted by maintaining B IS at about (50 ± 5) during operation, and the control group was adjusted according to the hemodynamic response, respiration and body movement in children The dosage of propofol, observed and recorded intraoperative circulation, B IS value, dosage of propofol, conscious extubation time and postoperative side effects. Results During the anesthesia and resuscitation, the blood pressure and B IS were higher in B IS group, the dosage of propofol was decreased, the time of extubation was shortened, and the incidence of postoperative nausea and vomiting was less. Conclusion BIS monitoring of propofol in children with tonsillectomy can maintain perioperative hemodynamic stability, reduce the dosage of propofol, fewer side effects, speed up the recovery of anesthesia.
