目的:了解某院西院区门诊药房近年来退药情况,分析退药原因,提出改进建议。方法:通过医院信息系统(HIS)收集某院西院区门诊药房2013年1月1日~2015年12月31日的退药信息,对药品名称、类别、数量、金额、退药原因等数据进行统计。结果:2013~2015年某院西院区门诊药房的退药处方数、退药药品金额、退药品种数均呈上升趋势;三年中,医院信息系统中最常见的退药原因分别为其他原因、医嘱停止和患者要求;退药金额与药品单价和退药数量呈正相关。结论:医院信息系统中退药原因选项的单一性,是造成以“其他原因”为退药原因比例逐年增加的主要因素。通过逐步完善医院信息系统、建立干预措施和强化药师专业素质等方法可以有效降低医院门诊药房的退药现象。
Objective: To understand the retreat situation of outpatient pharmacy in Xiyuan district of a hospital in recent years, analyze the reason of withdrawal and propose suggestions for improvement. Methods: The hospital information system (HIS) was used to collect the withdrawal information from outpatient pharmacy in Xiyuan district of a hospital from January 1, 2013 to December 31, 2015. The data of drug name, category, quantity, amount, withdrawal reason and so on statistics. Results: The number of retreat prescriptions, the amount of retreated drugs and the number of retreat drugs in outpatient pharmacies in Xiyuan District of a hospital increased from 2013 to 2015. The most common reasons for retreating in hospital information systems were other reasons for three years , The doctor’s order was stopped and the patient asked; the amount of drug withdrawal was positively correlated with the unit price of drugs and the amount of drug withdrawal. Conclusion: The singleness of the reasons for withdrawal from the hospital information system is the main factor that causes the proportion of “other causes” to be increased year by year. By gradually improving the hospital information system, establishing interventions and strengthening the professional quality of pharmacists, the withdrawal of drugs from the outpatient pharmacy can be effectively reduced.
