[目的]分析晚期非小细胞肺癌患者贫血发生的危险因素及贫血对预后的影响。[方法]统计41例晚期NSCLC患者治疗前后癌性贫血的发生率,分析其与性别、年龄、临床分期、PS评分的关系,并探讨癌性贫血对患者预后的影响。[结果]化疗2个周期后贫血发生率明显高于化疗前(63.4%vs24.4%,P<0.01),贫血严重程度随着化疗周期的增加而加重。单因素分析显示临床分期、PS评分是发生癌性贫血的危险因素;临床分期、PS评分、癌性贫血是影响患者无进展生存的因素,化疗前无贫血的患者较化疗前有贫血的患者中位无进展生存明显延长,分别为4.2个月和2.6个月(P=0.005)。临床分期、癌性贫血也是影响晚期NSCLC患者生存的危险因素,化疗后并发癌性贫血的患者中位生存期较未发生癌性贫血者明显缩短,分别为15个月和26个月(P=0.001)。[结论]晚期NSCLC患者有较高的贫血尤其是化疗相关性贫血的发生率,无论化疗前发生的贫血还是化疗导致的贫血均严重影响患者的无进展生存和总生存。
[Objective] To analyze the risk factors of anemia in patients with advanced non-small cell lung cancer and the influence of anemia on the prognosis. [Methods] The incidence of cancer anemia before and after treatment in 41 patients with advanced non-small cell lung cancer (NSCLC) was analyzed. The relationship between sex anemia, age, clinical stage and PS score was analyzed. The effect of cancerous anemia on prognosis was analyzed. [Results] The incidence of anemia after 2 cycles of chemotherapy was significantly higher than that before chemotherapy (63.4% vs 24.4%, P <0.01). The severity of anemia was aggravated with the increase of chemotherapy cycles. Univariate analysis showed that clinical stage and PS score were the risk factors for developing cancerous anemia. The clinical stage, PS score, and cancerous anemia were the factors influencing the progression-free survival of patients. Patients without anemia before chemotherapy had higher anemia than those without chemotherapy The progression-free survival was significantly longer, 4.2 months and 2.6 months, respectively (P = 0.005). Clinical stage and cancerous anemia were also risk factors for survival in patients with advanced NSCLC. The median survival of patients with cancerous anemia after chemotherapy was significantly shorter than that without cancerous anemia (15 and 26 months, P = 0.001). [Conclusion] The patients with advanced NSCLC have higher incidence of anemia, especially chemotherapy-related anemia. Both anemia caused by chemotherapy and chemotherapy-induced anemia seriously affect the patients’ progression-free survival and overall survival.
