目的探讨血清中同型半胱氨酸(homocysteine,HCY)与基质金属蛋白酶1(matrix metalloproteinases-1,MMP-1)水平评价扩张型心肌病(dilated cardiomyopathy,DCM)患者心功能的临床意义。方法收集作者医院2014-12/2015-08月DCM患者70例(实验组),并将其按心功能分级分为Ⅱ级20例、Ⅲ级28例、Ⅳ级22例,同期选取作者医院健康体检无DCM及其它器质性病变人员60例(对照组),分别检测外周血中HCY、MMP-1水平、左室射血分数(left ventricular ejection fractions,LVEF)及左室舒张末期内径(left ventricular end-diastolic diameter,LVEDD)。结果实验组HCY,MMP-1水平要高于对照组,两组比较差异具有统计学意义(P<0.05);随着心功能等级增加,HCY,MMP-1水平Ⅳ级>Ⅲ级>Ⅱ级,差异具有统计学意义(P<0.05);HCY的检测水平与LVEF呈负相关(r=-0.655,P<0.05),与LVEDD呈正相关(r=0.705,P<0.05);MMP-1的检测水平与LVEF呈负相关(r=-0.624,P<0.05),与LVEDD呈正相关(r=0.786,P<0.05)。结论DCM患者心功能等级越高,血浆中HCY,MMP-1水平越高,联合检测对评价DCM患者心功能损害有重要意义。
Objective To investigate the clinical significance of serum homocysteine (HCY) and matrix metalloproteinases-1 (MMP-1) in the evaluation of cardiac function in patients with dilated cardiomyopathy (DCM). Methods Seventy patients (experimental group) with DCM were enrolled in the hospital from 2014.12 / 2015/08. The patients were divided into two groups according to their cardiac function grading: 20 cases in grade Ⅱ, 28 cases in grade Ⅲ and 22 cases in grade Ⅳ. Sixty patients (control group) without DCM and other organic diseases were examined for HCY and MMP-1 in the peripheral blood, left ventricular ejection fractions (LVEF) and left ventricular end-diastolic diameter ventricular end-diastolic diameter, LVEDD). Results The levels of HCY and MMP-1 in the experimental group were significantly higher than those in the control group (P <0.05). With the increase of cardiac function, the levels of HCY and MMP-1 were higher than those of the control group , The difference was statistically significant (P <0.05). The detection level of HCY was negatively correlated with LVEF (r = -0.655, P <0.05), positively correlated with LVEDD (r = 0.705, The detection level was negatively correlated with LVEF (r = -0.624, P <0.05), and positively correlated with LVEDD (r = 0.786, P <0.05). Conclusions The higher the level of cardiac function in DCM patients, the higher the level of HCY and MMP-1 in plasma. The combined detection of cardiac function in patients with DCM is of great significance.
