目的:探讨心大静脉闭塞对心脏起搏阈值的短期与长期影响。方法:24头家猪采用经皮放置金属弹簧圈,建立心大静脉闭塞动物模型,分别测定闭塞前和闭塞后1h、1~3周右心室心尖部、左心室游离壁、左心室间隔部以及左心室心尖部起搏阈值;4头家猪用经皮经腔冠状动脉成形术球囊阻塞心大静脉建立心大静脉阻塞模型,测定阻塞前和阻塞后5、15、25、35、45、60min心大静脉内起搏阈值。结果:心大静脉闭塞后,左心室间隔部、心尖部起搏阈值明显升高(均P<0.01),左心室游离壁起搏阈值无明显变化。心大静脉阻塞60min内,局部起搏阈值差异无统计学意义(均P>0.05)。结论:心大静脉闭塞可导致邻近部位心肌起搏阈值升高,但心大静脉内局部起搏阈值在阻塞后1h内无明显变化。
Objective: To investigate the short-term and long-term effects of cardiac ventricular occlusion on cardiac pacing threshold. Methods: Twenty-four domestic pigs were percutaneously placed with metal coil. The animal model of ventricular occlusion was established. The right ventricular apex, left ventricular free wall, left ventricular septum, and left ventricular septum before and 1 and 1, Left ventricular apical pacing threshold; 4 domestic pig percutaneous transluminal coronary angioplasty balloon occlusion of the great heart vein occlusion model was established before and after occlusion occlusion 5,15,25,35,45, 60min cardiac pacing threshold. Results: Left ventricular septal and apical pacing thresholds were significantly increased (all P <0.01) after left ventricular occlusion, and there was no significant change in left ventricular free wall pacing threshold. There was no significant difference in local pacing threshold within 60 minutes of occlusion of cardiac vein (all P> 0.05). CONCLUSION: Ventricular major venous occlusion may lead to an increase of myocardial pacing threshold in the proximal part. However, the local pacing threshold in the ventricular dural vein does not change significantly within 1 h after occlusion.
