病例资料 我科1998年5月至2008年10月期间共收治严重肝外伤患者12例,均为男性,年龄9～56岁,平均29岁.12例均为闭合伤,其中挤压伤5例,重物撞击伤4例,高处跌落伤3例.合并多发肋骨

病例资料 我科1998年5月至2008年10月期间共收治严重肝外伤患者12例,均为男性,年龄9～56岁,平均29岁.12例均为闭合伤,其中挤压伤5例,重物撞击伤4例,高处跌落伤3例.合并多发肋骨骨折3例,血气胸3例,肾挫伤2例,后腹膜血肿2例,颅脑外伤1例.12例患者根据右上腹部或右下胸背部外伤史、内出血症状、腹膜炎体征及腹腔穿刺抽出不凝血均确诊为“外伤性肝破裂”,并于入院后在快速液体复苏的同时行急诊剖腹探查术.术中发现:肝右叶损伤7例,肝左叶损伤2例,左右叶均损伤3例;合并肝左静脉离断伤1例,肝后下腔静脉裂伤1例,肝中央区严重损伤1例.按Moore分类法:Ⅲ级5例、Ⅳ级3例和Ⅴ级4例.
