目的报告2例疫苗相关麻痹型脊髓灰质炎病例(VAPP),为疫苗接种不良反应的处置提供参考。方法采集患儿双份粪便标本,由福建省CDC进行病毒分离,阳性标本送国家CDC鉴定。结果 2例均为出生2月龄男童,均首次接种OPV。病例1服苗23d出现症状,60d后残留双下肢麻痹,鉴定为Ⅰ+Ⅱ型脊灰疫苗相关株;病例2服苗26d出现症状,60d后右下肢残留麻痹,鉴定为Ⅲ型脊灰疫苗相关株,患儿均由省财政给予相应补偿。结论对接种首剂OPV的儿童,应认真询问健康状况,对健康状况不佳者应推迟接种或建议使用IPV;加强AFP监测,及时发现和治疗VAPP,减少并发症。
Objective To report two cases of vaccine-associated paralytic poliomyelitis (VAPP) and provide reference for the treatment of adverse reactions of vaccination. Methods Duplicate stool samples were collected from children with CDC virus isolation, the positive specimens sent to the national CDC identification. Results Both 2-month-old boys and 2-year-old boys were vaccinated with OPV for the first time. Symptoms were observed on the 23rd day after treatment in the case 1, and paralysis of both lower extremities left after 60 days was identified as type I + II poliovirus vaccine. Case 2 served as a symptom on the 26th day and remained paralyzed on the right leg 60 days later, which was identified as type III poliovirus vaccine Strain, children are given by the provincial finance corresponding compensation. Conclusions Children who received the first OPV should be asked their health status carefully, postpone their vaccination or IPV recommended for those with poor health status, strengthen AFP surveillance, detect and treat VAPP in time, and reduce complications.
