目的探讨血清总同型半胱氨酸(tHcy)、叶酸、VitB12与老年男性骨密度的关系.方法用化学发光免疫法测定101例60岁以上男性血清tHcy、叶酸和VitB12的浓度,用双能X线吸收测量法(DEXA)测定腰椎前后位总体及股骨总体的骨密度(BMD),根据T≤-1和T＞-1分成骨量低下(含骨质疏松)和骨量正常两组.结果(1)腰椎前后位总体及股骨总体骨量低下组血清叶酸水平均低于骨量正常组(P分别为0.000,0.036),血清tHcy、VitB12水平两组间差异无显著性(P＞0.05).(2)腰椎前后位总体和股骨总体BMD与体重(r=0.204,P=0.041;r=0.020,P=0.048)、BMI(r=0.258,P=0.009;r=0.280,P=0.005)、叶酸(r=0.636,P=0.000;r=0.284,P=0.004)呈正相关;与tHcy(r=-0.264,P=0.008;r=-0.202,P=0.043)、AKP呈负相关(r=-0.201,P=0.044;r=-0.228,P=0.022);与年龄,血清VitB12、钙、磷水平没有相关性.(3)腰椎前后位总体及股骨总体BMD的影响因子是BMI、血清叶酸水平.结论老年男性腰椎前后位总体及股骨总体的骨密度下降与血清叶酸水平降低有关.
