1986年1月~1992年12月,我院共收治宫颈复发癌36例,本文对临床资料进行回顾性分析,探讨复发时间、有关因素、诊断和治疗方法。资料显示复发间隔时间平均1.1年,2年内复发29例(81%)。原发癌Ⅱb~Ⅳ期有30例(83.3%)。分化差的鳞癌、腺鳞癌、腺癌30例(83.3%)。结节型22例(60%)。颈管增粗肿瘤直径大于4cm16例。原发癌均有病理证实;复发癌有病理证实15例,由影像学证实14例,仅依据阴道排液、疼痛加重、下肢肿大、发现肿块等临床症状、体征诊断7例。治疗施用放疗或/和化疗,随访5~11年,随访率94.4%,全组中位生存期17个月,5年存活率8.3%。本文认为宫颈癌首次治疗后一定要门诊随访,尤其2年内必须密切定期随访。复发与临床分期、病理类型及肿瘤类型有关,尤与宫颈肿瘤大小关系密切。复发癌诊断困难,治疗上因人而异,首次放疗宜手术切除,首次手术应以放疗为主,化疗也是十分重要的手段,如不失治疗时机仍可延长部分患者寿命。

Primary cancer were pathologically confirmed; recurrence of cancer pathologically confirmed in 15 cases, confirmed by imaging 14 cases, only based on vaginal discharge, increased pain, lower extremity swelling, found the tumor and other clinical symptoms and signs of diagnosis in 7 cases. The patients were treated with radiotherapy and / or chemotherapy. The follow-up was 5 to 11 years. The follow-up rate was 94.4%. The median survival time was 17 months and the 5-year survival rate was 8.3%. This article believes that after the first treatment of cervical cancer must be outpatient follow-up, in particular, must be close regular follow-up within 2 years. Recurrence and clinical stage, pathological type and tumor type, especially with the size of cervical cancer are closely related. The diagnosis of recurrent cancer is difficult, the treatment varies from person to person, the first radiotherapy should be surgically removed, the first surgery should be based on radiotherapy, chemotherapy is also very important means, such as the timing of treatment can extend the life of some patients.
