2型糖尿病患者社区全科模式下健康管理干预临床观察

目的:探讨2型糖尿病患者在社区全科模式下行健康管理干预的效果。方法:选择广州医科大学附属口腔医院2014年6月至2015年6月2型糖尿病患者120例,随机分为观察组与对照组,各60例,对照组采取常规护理,观察组在此基础上采取社区全科模式健康管理法,比较两组患者经过干预后对糖尿病知识的认知度、血糖水平与护理满意度。结果:(1)与对照组(发病原因41.67%、临床症状50.00%、诊断标准33.33%、高危因素50.00%、并发症58.33%、低血糖反应50.00%)比较,观察组经过干预后对糖尿病知识的认知度(发病原因91.67%、临床症状93.33%、诊断标准83.33%、高危因素88.33%、并发症93.33%、低血糖反应91.67%)均明显更高,差异具有统计学意义(P<0.05)。(2)与对照组对比,观察组糖化血红蛋白、空腹血糖、餐后2 h血糖水平明显更低,差异具有统计学意义(P<0.05)。(3)与对照组75%对比,观察组护理满意率95%明显更高,差异具有统计学意义(P<0.05)。结论:2型糖尿病患者在社区全科模式下行健康管理干预的效果显著,有效地提高患者对糖尿病知识的认识度,提高护理满意率,并且有利于患者对血糖的监控,从而促进血糖水平降低。 Objective: To investigate the effect of health management intervention in type 2 diabetes patients in community general practice. Methods: One hundred and twenty patients with type 2 diabetes mellitus were selected from Affiliated Stomatological Hospital of Guangzhou Medical University from June 2014 to June 2015. They were randomly divided into observation group and control group, with 60 cases in each group. The control group received routine nursing. On the basis of this observation group Community health management approach was adopted to compare the knowledge of diabetes, blood glucose and nursing satisfaction after intervention. Results: (1) Compared with the control group (incidence of 41.67%, clinical symptoms 50.00%, diagnostic criteria 33.33%, risk factors 50.00%, complications 58.33%, hypoglycemia 50.00%), the observation group after intervention on the knowledge of diabetes (91.67%, 93.33% of clinical symptoms, 83.33% of diagnostic criteria, 88.33% of risk factors, 93.33% of complications, and 91.67% of hypoglycemic reactions) were significantly higher (P <0.05) ). (2) Compared with the control group, HbA1c, fasting blood glucose and postprandial 2-h blood glucose levels were significantly lower in the observation group, with statistical significance (P <0.05). (3) Compared with the control group of 75%, the satisfaction rate of the nursing group was significantly higher 95%, the difference was statistically significant (P <0.05). CONCLUSION: The effect of health management intervention in patients with type 2 diabetes mellitus is significant, which can effectively improve patients’ awareness of diabetes, improve nursing satisfaction, and help patients to monitor their blood glucose, thus contributing to the reduction of blood glucose.
