目的:探讨含ω-3多不饱和脂肪酸(PUFA)的肠外营养对胃癌病人免疫功能的影响。方法:计算机检索1996年1月至2014年5月国内外数据库中研究择期手术后补充ω-3 PUFA对胃癌病人免疫功能影响的文献,同时进行手工检索和参考文献回溯。采用偏倚风险评估标准对纳入随机对照试验的质量进行评价。采用RevMan 5.1软件进行Meta分析,研究ω-3 PUFA对胃癌病人免疫功能(Ig A,Ig G,Ig M,CD4,CD8,CD4/CD8)的影响。结果:共检索到1 176篇相关文献,剔除数据重复、抄袭和无法获得方法学资料的文摘,最终纳入5个随机对照研究,共计217例病人。Meta分析结果显示,含ω-3 PUFA的肠外营养与对照组比,CD4/CD8、CD4、Ig G、Ig A、Ig M等差异均有统计学意义。结论:适当剂量和一定干预时间的ω-3 PUFA能增强胃癌病人应激状态下的免疫功能。
Objective: To investigate the effect of parenteral nutrition containing omega-3 polyunsaturated fatty acids (PUFA) on the immune function of gastric cancer patients. Methods: We searched the databases of domestic and international databases from January 1996 to May 2014 for the study of the effects of omega-3 PUFA supplementation on immune function in patients with gastric cancer after elective surgery, and searched the manuals and retrospectively. The quality of included randomized controlled trials was assessed using the bias risk assessment criteria. Meta-analysis was performed using RevMan 5.1 software to investigate the effect of omega-3 PUFA on immune function (IgA, IgG, IgM, CD4, CD8, CD4 / CD8) in gastric cancer patients. RESULTS: A total of 1 176 articles were retrieved, abstracts of data duplication, plagiarism and inability to obtain methodological data were culled. Finally, five randomized controlled studies were included, for a total of 217 patients. The result of Meta analysis showed that there was significant difference in the ratio of CD4 / CD8, CD4, Ig G, Ig A and Ig M between the omega-3 PUFA-containing parenteral nutrition group and the control group. CONCLUSION: Omega-3 PUFA at appropriate dosage and intervention time can enhance the immune function in gastric cancer patients under stress.
