目的探讨中性粒细胞CD64的表达在手足口病患者并发细菌感染中的诊断价值。方法选择手足口病并确诊为并发细菌感染的住院患儿38例,无细菌感染患儿52例,应用流式细胞术测定中性粒细胞CD64,同时测外周血C-反应蛋白(CRP)、白细胞及分类进行比较。结果并发感染组治疗前CD64水平为(14.51±6.07)MFI,明显高于治疗后组的(10.6±3.23)MFI及未感染治疗前组(9.55±3.48)MFI,差异有统计学意义(t=5.7、3.4,P<0.05);根据ROC曲线,以CD64≥11.3 5MFI为阳性标准,其诊断并发细菌感染的敏感度、特异度和准确度分别为82.25%、78.91%及81.32%。结论中性粒细胞CD64的表达可作为手足口病并发细菌的检测指标。
Objective To investigate the diagnostic value of neutrophil CD64 expression in patients with hand-foot-mouth disease complicated by bacterial infection. Methods Thirty-eight inpatients with hand-foot-mouth disease and confirmed bacterial infection were included in this study. Fifty-two children without bacterial infection were selected. Flowcytometry was used to determine neutrophil CD64 and peripheral blood C-reactive protein (CRP) Leukocytes and classification were compared. Results The level of CD64 in pre-treatment group was (14.51 ± 6.07) MFI, which was significantly higher than that of MFI group (10.6 ± 3.23) and MFI group (9.55 ± 3.48) before treatment (t = 5.7, 3.4, P <0.05). According to the ROC curve, the sensitivity, specificity and accuracy of CD64≥11.3 5 MFI for the diagnosis of concurrent bacterial infection were 82.25%, 78.91% and 81.32%, respectively. Conclusion The expression of neutrophil CD64 can be used as a marker of hand-foot-mouth disease complicated with bacteria.
