目的探讨高危型人类乳头瘤病毒(human papillomavirus,HPV)分型检测在宫颈疾病诊断中的临床价值。方法选取2013年12月至2015年12月于朝阳市中心医院妇科行宫颈疾病病理检查的1436例妇女作为研究对象,根据病理检查结果将其分为对照组(宫颈未见异常)1046例和观察组(确诊为宫颈疾病)390例。观察组按疾病类型分为宫颈炎(153例)、宫颈上皮内瘤变(CIN)Ⅰ~Ⅱ级(161例)、CINⅢ(47例)以及宫颈癌(29例)。所有研究对象均行HPV分型检测,比较观察组与对照组研究对象高危型HPV感染情况,并分析高危型HPV感染与宫颈疾病的相关性。结果观察组与对照组研究对象均可出现HPV感染,且均以单一高危型HPV感染为主,但观察组患者高危型HPV感染率显著高于对照组,差异有统计学意义(P<0.05);应用Spearman分析进行高危型HPV感染与宫颈病变之间的相关性,相关系数r为0.821,χ2值为29.754,(P<0.01),具有显著正相关。结论宫颈病变程度越严重,高危型HPV感染率越高,因此高危型HPV分型检测可用于准确评估宫颈病变严重程度。
Objective To investigate the clinical value of high-risk human papillomavirus (HPV) typing in the diagnosis of cervical diseases. Methods From December 2013 to December 2015 in Chaoyang Central Hospital gynecological cervical disease pathological examination of 1436 women as the study object, according to the pathological findings were divided into control group (no abnormal cervical) 1046 cases and observed Group (diagnosed as cervical disease) 390 cases. According to the type of disease, the observation group was divided into cervicitis (153 cases), cervical intraepithelial neoplasia (CIN) Ⅰ ~ Ⅱ grade (161 cases), CIN Ⅲ (47 cases) and cervical cancer (29 cases). All subjects underwent HPV genotyping. The high-risk HPV infection in the observation group and the control group were compared, and the correlation between high-risk HPV infection and cervical disease was analyzed. Results HPV infection was observed in the observation group and the control group, all of which were single high-risk HPV infection, but the infection rate of high-risk HPV in the observation group was significantly higher than that of the control group (P <0.05) ; Spearman analysis of high-risk HPV infection and cervical lesions correlation, the correlation coefficient r was 0.821, χ2 value of 29.754, (P <0.01), with a significant positive correlation. Conclusion The more severe the cervical lesions are, the higher the infection rate of high-risk HPV is. Therefore, high-risk HPV typing can be used to accurately evaluate the severity of cervical lesions.
